Submission and Documentation Requirements:

1. Complete the online Rebate Redemption Form

2. Verify that all data is entered correctly

3. Print and submit an on-line Rebate Redemption Form (Rebate4U.com)

4. Print and submit the Microsoft Windows^®8 Upgrade receipt

5. Submit the ORIGINAL Lenovo PC sales receipt or invoice and cut and submit the entire UPC label off of the box.

NOTE: If you have previously submitted your original documentation for Lenovo’s Labor Day Promotion, you will only be required to submit:

1. A copy of your original Labor Day Rebate Redemption Form, and

2. A copy of the Windows 8 Upgrade receipt

6. MAINTAIN A PHOTOCOPY OF ALL DOCUMENTS
