1. This offer is valid only with the purchase of Motorola RDX Series™, Motorola CLS Series™ or Motorola DTR410™ two-way radios between 3/1/2009 and 6/30/2009. Offer excludes CLS1000, CLS1450c and all bulk (non-packaged) purchase models.
2) To receive your rebate, you must completely fill out the online submission form (available at www.motorolasavings.com, on website simply have a click here function that directs to form). You will receive a confirmation page at the end of online submission process. Finalize your rebate submission by mailing in the following:
- Signed printout of completed online submission form confirmation page
- A copy of purchase receipt (packing slips not accepted)
- 1”x 3” barcode from product packaging for all radios purchased.
Send your rebate submission package to the address below. Redemption requests must be postmarked on or before 7/15/2009, and received by 7/31/2009.

A minimum of 6 radios within the same product series must be purchased to qualify for a rebate – multiple orders count toward redemption. Mixing between product series (RDX Series™, CLS Series™, DTR410™) is not allowed for reaching a multiple of 6 radios purchased. .
Maximum of 8 total redemptions (only 48 radios purchased eligible) per business location. Maximum redemption allowed is either $1,600 for RDX and DTR series purchases or $800 for CLS series purchased..
5. Offer good only in the U.S., Canada and Puerto Rico and is void where prohibited, taxed or restricted by law.
6. Motorola RDX Series™ or CLS Series™ or DTR410™ two-way radio(s) cannot be returned once this original rebate submission form has been submitted.
7. Not responsible for lost, late, mutilated, misdirected or postage due mail. Illegible or incomplete forms will be considered invalid and ineligible for offer.
8. Photocopies or mechanical reproductions of the 1” x 3” barcodes from product packaging will not be accepted.
9. This offer may not be combined with any other promotional offers, rebates, coupons or discounts.
10. Fraudulent submissions could result in prosecution under U.S. Mail Fraud Statute (18 USC Section 1241-1242).
11. This form has no cash value. Reproduction, purchase, sale or trade of 1” x
3” barcodes from product packaging is prohibited.
12. Materials received become the property of Motorola Inc. and cannot be returned.
