Minitor V Pager Rebate Terms & Conditions

 1. This offer of $75 rebate is valid only with the purchase of Minitor™ V Pagers between April 10, 2008 and September 15, 2008. Minimum purchase of 5 pagers to qualify.

2. To receive the rebates, completely fill out the online submission form (available at www.motorolapagerpromo.com). Customer will receive a confirmation page at end of online submission process. To finalize the rebate submission process, mail in the following:

- Signed printout of completed online submission form confirmation page
- A copy of purchase receipt (packing slips not accepted)
- Eligible trade-in radio units

Rebate submissions must be postmarked on or before September 30, 2008 and received by October 15, 2008.

3. Redemptions cannot be sent to P.O. Boxes. For questions, please call 1-866-668-7271 or visit www.motorolapagerpromo.com. Please allow 4-6 weeks from receipt of your submission form to receive you rebate.

4. Pagers eligible for trade-in include: Minitor II, Minitor III, Minitor IV, US Alert Nova, SCA Alps, SCA Sceptar Swissphone RE529, Icom F50, Apollo VP100, and Apollo VP200.

All other brands and models of pagers are excluded from this rebate offer.

5. Offer only valid in the United States. Not applicable for Canada, Puerto Rico and Guam. Void where prohibited, taxed or restricted by law.

6. Motorola Minitor V Pagers cannot be returned once the original rebate submission form has been submitted.

7. Not responsible for lost, late, mutilated, misdirected or postage due mail. Illegible or incomplete forms will be considered invalid and ineligible for offer.

8. This offer may not be combined with any other promotional offers, rebates, coupons or discounts.

9. Fraudulent submissions could result in prosecution under U.S. Mail Fraud Statute (18 USC Section 1241-1242).

10. This form has no cash value. Reproduction, purchase, sale or trade of rebate submission material is prohibited.

11. Materials received become the property of Motorola Inc. and cannot be returned.

12. Motorola reserves the right to modify or withdraw this promotion at any time, without any prior notice. Please make a copy of all materials submitted.
