How to Fill Out the Rebate Redemption Form
Consumers that have purchased a qualifying Rheem HVAC system can receive their rebate by filling out a Rebate Redemption form and mailing the confirming documents to our rebate processing center at:

ON-LINE REDEMPTION FORM
The Rebate Redemption Form is located at www.rheemrebate.com. Consumers will be required to enter the following information to claim their rebate:
· Consumer Name
· Address
· Phone Number
· Contractor Name and Phone Number
· Model Number and Serial Number (if applicable) of qualifying equipment
o Outdoor Unit (A/C, HP, or Package Unit)
o Indoor Unit (Furnace or Air Handler)
o Thermostat
o Filter Rack (If applicable)
After filling out the form, the system will calculate the Consumer’s rebate based on the system information entered.
The Consumer will then print out a confirmation sheet that summarizes their rebate, sign and date it and mail it and a copy of the itemized invoice from the installing contractor to the rebate processing center address above.
MANUAL REDEMPTION FORM ENTRY
Consumers that do not want to fill out their Rebate Redemption form on-line can print out a form and fill it out manually. They will still need to provide all of the information detailed above and will have to mail the form and a copy of the itemized invoice from the installing contractor to the rebate processing center.
